<p style="text-align: justify;"><span id="more-345"></span>&#8220;More than 75 percent of children with cancer are enrolled by their parents at some point in their treatment,&#8221; she said. &#8220;Even though there is a much smaller number of children than adults with cancer, there are so many of them in clinical trials that there&#8217;s a big enough sample size to make a lot of statistically significant findings.&#8221;</p>
<p style="text-align: justify;">For example, Keene said, 30 years ago childhood leukemia was almost always fatal. Now, more than 80 percent of kids with acute leukemia survive.</p>
<p style="text-align: justify;">Keene ascribes the large number of children in trials to motivation. &#8220;Parents will do anything to save their children,&#8221; she said.</p>
<p style="text-align: justify;">If the child is old enough and willing to be involved, sometimes parents will include the children in the discussion about the trial and have them sign the consent form as well. If the trial involves a teen-ager, and there is disagreement between the teen and his parents, the resolution may be handled differently by each institution. Often, a medical center&#8217;s ethics committee gets involved, the teen or the parents may get representation, and, at some point, everyone hopefully sits down together and comes to a decision that&#8217;s mutually acceptable. &#8220;I don&#8217;t think the situation happens very often, but when it does, it is emotionally wrenching,&#8221; Keene said.</p>
<p style="text-align: justify;">There are many considerations for adults who are thinking about joining a clinical trial. Some are uncomfortable about the lack of control over which arm of an experiment they end up in. For example, in a randomized study, you or your doctor can&#8217;t decide whether you&#8217;ll get the standard treatment or the experimental one. A computer decides this.</p>
<p style="text-align: justify;">Trials also can involve more treatments, hospital visits, checkups and discomfort. There is a question of traveling and, in some cases, who is going to pay for it.</p>
<p style="text-align: justify;">The option of doing a clinical trial &#8220;makes decisions a little more complicated. Otherwise, all you&#8217;d have to consider are the standard treatments,&#8221; Finn said.</p>
<p style="text-align: justify;">For example, in one of the trials run by Madajewicz, a new combination of drugs to treat advanced colorectal cancer is being evaluated against the treatment that has been the standard for several years. Once the safe dosage levels have been determined in phase one, the trial moves to phase two, where the efficacy of the new treatment is studied.</p>
<p style="text-align: justify;">&#8220;Our goal in this particular trial is that we get at least a 15 percent better result than is seen with the standard treatment and that we can significantly prolong a patient&#8217;s survival,&#8221; said Madajewicz.</p>
<p style="text-align: justify;">Of course, trials are not only about cancer, and they are not only about treatment. They are done for a variety of medical issues, for alternative or complementary medicine, and for prevention.</p>
<p style="text-align: justify;">Patricia Hentschel is a nurse practitioner and project director at the Stony Brook branch of STAR, a nationwide study of more than 20,000 women to compare the benefits of tamoxifen and raloxifene in the prevention of breast cancer in women who are at high risk.</p>

<p style="text-align: justify;"><span id="more-342"></span>What can happen, Finn said, is that a person has a serious illness, is very worried and hears that there is an experimental treatment under investigation. Handed an informed consent form, the patient may be quick to say, &#8220;OK, doc, where do I sign?&#8221; Finn said. Instead of rushing into it, he said to read the informed consent form very carefully, ask lots and lots of questions, think it over, talk to knowledgeable family members and friends, and consider getting a second medical opinion.</p>
<p style="text-align: justify;">In the pull between the needs of research and those of the patient, you are still your own best advocate. Given the available options, being wary and well-informed can keep you on a decision-making course that is best for your own health.</p>
<p style="text-align: justify;">&#8220;Do you want to participate in this trial, or another trial? Do you want to get the standard treatment? Or do you want to get no treatment at all? These are all valid, rational options,&#8221; Finn said.</p>
<p style="text-align: justify;">For some patients, the problem is how to find out about clinical trials that might be beneficial for them. &#8220;Even if you have the best oncologist in the world, he or she may know about a couple of dozen clinical trials,&#8221; Finn said. The National Cancer Institute (NCI) runs about 1,700 trials at any given time and pharmaceutical companies probably have at least that number. Some doctors, especially those who are not affiliated with any major medical centers, may not be in the loop of the latest trial opportunities.</p>
<p style="text-align: justify;">&#8220;Way too few people consider cancer clinical trials,&#8221; he said. For example, estimates are that between 2 and 5 percent of adults with cancer participate in clinical trials.</p>

And if there isn&#8217;t a solution at the moment, we like to think that one is just around the corner.</p>
<p style="text-align: justify;"><span id="more-339"></span> But getting from point A, the disease, to point B, the treatment, takes a giant step. And that includes doing tests on human beings.</p>
<p style="text-align: justify;">Such tests, called clinical trials, are the only way to advance medicine, said Dr. Stefan Madajewicz, director of clinical trials in oncology at the State University of New York, University Hospital and Medical Center at Stony Brook, N.Y. &#8220;There are many ideas in the medical and paramedical communities. But the only proof of value in a treatment is through clinical trial. It&#8217;s not based on individual or group beliefs. It&#8217;s based on hard, cold mathematical data.&#8221;</p>
<p style="text-align: justify;">For some, the idea of doing experimentation that could put people at risk smacks of human beings turned guinea pigs. And some recent experiments, including one in gene therapy that resulted in the death of a participant, have raised fears anew of the dangers of testing out treatments on people.</p>
<p style="text-align: justify;">However, said Robert Finn, author of &#8220;Cancer Clinical Trials,&#8221; (O&#8217;Reilly, 1999, $14.95) &#8220;Participating in a clinical trial is the opposite of being a guinea pig. Experimental animals have no choice about whether to participate in an experiment and no ability to leave the it.&#8221;</p>
<p style="text-align: justify;">Humans, on the other hand, decide which trial to participate in and, once enrolled, can leave at any time, he said. And that means whether you don&#8217;t like the side effects, you didn&#8217;t get into the study arm you preferred, you&#8217;ve found a better treatment option, or you just want out.</p>
<p style="text-align: justify;">But the tougher decision for some is not when to stop, but whether to begin. And since there are thousands of clinical trials going on at any given time, you may well have to make that decision for yourself or for a family member.</p>
<p style="text-align: justify;">&#8220;I believe in the value of clinical trials, and I believe in the family&#8217;s right to make their own decision, without pressure,&#8221; said Nancy Keene, who enrolled her young daughter in a clinical trial for children with leukemia. &#8220;Doctors and parents should work together, with the understanding it&#8217;s a free choice.&#8221; If a clinical trial is an option, it has to be weighed carefully against the choice of continuing with the standard treatment.</p>
<p style="text-align: justify;">Keene, the co-author of &#8220;Childhood Cancer,&#8221; (O&#8217;Reilly, 1999, $24.95) went through an intense process of uncovering information as she made her decision. She said she talked so much that the doctor wrote in her daughter&#8217;s chart, &#8220;Mother asks innumerable questions.&#8221;</p>
<p style="text-align: justify;">Still, with only 72 hours to decide, she felt she didn&#8217;t have enough information. The particular trial she was evaluating was to find out whether you could treat leukemia as well and do less damage to the brain by eliminating radiation and giving a child more chemotherapy.</p>
<p style="text-align: justify;">She said she signed the consent form in time to enroll her child, knowing that in the first month of the trial there would be no differences in treatment and she could continue to do research. &#8220;My family and friends helped me do a huge literature search. I read and read and read.&#8221; By the time the randomized part of the study began &#8212; some children getting radiation and some getting more chemotherapy &#8212; Keene said she decided to go ahead. &#8220;If I didn&#8217;t think it was in my child&#8217;s best interest, I would have withdrawn.&#8221;</p>

<p style="text-align: justify;">As medical officials in South Florida cope with a third case of anthrax exposure, more Americans are worrying about the simple act of breathing.</p>
<p style="text-align: justify;">Gas masks are in short supply, a drug company is increasing production of a newly popular antibiotic, and scary books about germ attacks are flying off shelves.</p>
<p style="text-align: justify;"><span id="more-251"></span>Is there anything that average citizens can do to protect themselves from the airborne strain of this bacteria as they go to work, shop at malls and attend football games? The answer, according to experts, is no.</p>
<p style="text-align: justify;">&#8220;There&#8217;s absolutely nothing that people should do. Pulling off a large-scale terrorist activity would be extremely difficult and would take a lot sophistication, money and technical expertise,&#8221; said Keith S. Kaye, an epidemiologist at Duke University. There are, he said, easier ways to kill large numbers of people.</p>
<p style="text-align: justify;">Others aren&#8217;t sure the risk is that low, but they agree that there&#8217;s little that can be done other than simple disaster preparedness &#8212; keeping food, water and a radio handy if you need to stay indoors.</p>
<p style="text-align: justify;">Clearly, anthrax can attack without warning and not be noticed for days. Florida medical officials announced lat yesterday that a third person, a 35-year-old woman, had been exposed to anthrax at a Boca Raton building that houses supermarket tabloids. She is expected to survive, as is the second exposed person, a 73-year-old mail room supervisor.</p>
<p style="text-align: justify;">Bob Stevens, a 63-year-old photo editor at The Sun newspaper, died last week of a rare form of inhaled anthrax. Investigators found anthrax spores on his computer keyboard, but no one knows where they came from.</p>
<p style="text-align: justify;">Although U.S. tabloids have in print ravaged terrorist suspect Osama bin Laden, the FBI said there&#8217;s no evidence that the outbreak was related to the Sept. 11 attacks.</p>
<p style="text-align: justify;">The degree of difficulty in turning anthrax into a weapon is somewhere between easy and difficult, said Mike Sailor, a professor of chemistry and biochemistry at the University of California at San Diego.</p>
<p style="text-align: justify;">He speculated that someone with a college education could do it and added that the process is similar to that of growing yeast to make beer.</p>
<p style="text-align: justify;">The main ingredient, of course, is anthrax. Spores can be found in virtually any cow field, he said. &#8220;It&#8217;s a naturally occurring disease of cattle and sheep. The reason everybody isn&#8217;t dropping dead is that it isn&#8217;t floating in the air, you aren&#8217;t breathing in 8,000 spores.&#8221;</p>
<p style="text-align: justify;">Without advance warning, there are few effective ways to protect against an anthrax attack, experts said. People wouldn&#8217;t know they were exposed until someone developed the disease a few days later. The most nightmarish scenarios &#8212; such as one in a 2000 book about biological attacks called Living Terrors &#8212; envision a cloud of anthrax spores landing invisibly on a stadium full of sports fans.</p>
<p style="text-align: justify;">So what to do? One option would be to stock up on an antibiotic called ciprofloxacin, which is better known as Cipro. With some reservations, the federal government has approved it for use against inhalation anthrax, the extremely rare strain that killed the Florida photo editor.</p>
<p style="text-align: justify;">Demand for Cipro has skyrocketed in recent weeks, and the company that makes it, Bayer Inc., is boosting production by 25 percent, said spokesman Rob Kloppenburg. Cipro, which has been on the market about 14 years, makes an estimated $1.5 billion in sales a year, and treats a variety of diseases, including urinary tract infections.</p>
<p style="text-align: justify;">But the drug is expensive &#8212; about $10 a day at wholesale rates &#8212; and it only works if treatment begins a short time after anthrax exposure. It has side effects, and liberal use of the <a href="http://generics-one.com/">generic medication</a> could boost drug-resistant germs.</p>
<p style="text-align: justify;">&#8220;People are buying it because nobody they respect has told them, &#8216;Don&#8217;t worry, we will come to your aid, we will provide the drugs in a timely manner, we are ready,&#8217;&#8221; said Dr. Mohammad N. Akhter, executive director of the American Public Health Association. &#8220;People need to hear that message.&#8221;</p>
<p style="text-align: justify;">Some people are turning to gas masks. They will ward off anthrax if they have the proper filters, said Sailor. But he doesn&#8217;t recommend them because terrorists could turn to other biological or chemical agents instead.</p>
<p style="text-align: justify;">&#8220;There are many different kinds of scenarios,&#8221; he said. &#8220;If everyone&#8217;s wearing a gas mask, and they spray a nerve agent, it&#8217;s going to get on your skin and kill you. People walking around in gas masks would be very cumbersome and give you a false sense of security.&#8221;</p>
<p style="text-align: justify;">Akhter suggests that people do one thing: watch the world around them. &#8220;The citizens just need to be vigilant,&#8221; he said. &#8220;If they see an unusual incident or something out of the ordinary, they should report it to the authorities. Besides that, they don&#8217;t need to do anything.&#8221;</p>

But a study in the September 14 Archives of Internal Medicine suggests hormone therapy may have a potentially troublesome effect: It can make cholesterol levels fluctuate throughout a hormone cycle. <span id="more-205"></span></p>
<p>Researchers recruited 36 women for the study; all had high cholesterol and were on hormone therapy – the kind in which the first half of the cycle is estrogen only and the second half is combined estrogen-plus-progestin. The researchers tracked the women&#8217;s cholesterol levels for three cycles. </p>
<p>The study found that LDL (bad) cholesterol jumped from an average of 156 mg/dl after the estrogen-only phase (day 13 of the cycle) to 178 after the combined phase (day 25). This is a large change, probably caused by the opposite effects of estrogen and progestin on cholesterol levels. </p>
<p><a href="http://www.buydrugsnoprescription.com/list-of-cholesterol-generic-drugs.html">Such fluctuations could make it hard for a woman to accurately track these levels. That&#8217;s because her readings could reflect not just her diet and exercise habits but also the times of the month she was tested. </a></p>
<p>So the study authors have this advice: If you&#8217;re a postmenopausal woman taking hormones and it&#8217;s time for your regular cholesterol test, get two – one during the estrogen-only phase of your pills, and one during the combined phase of the same cycle. If these two measurements differ by more than a few points, use the less favorable one as your benchmark, and make sure your doctor does all future cholesterol tests during that phase.</p>
